Please also remember to bring your driver's license or photo ID, insurance card if applicable, a complete list of all your medications plus any x-rays, EMG's or other relevant diagnostic study reports and/or copies of your medical records. The faster we can get your paperwork taken care of, the faster we can take care of your pain.
